Do I qualify?
You qualify for PACE if you meet the following criteria:
- At least 55 years old
- Are qualified by the state as being eligible for nursing facility level of care
- Are able to live safely at home and in the community
- Are living in the CV PACE service area

Day Center
The hub for all of our medical services and activities, you’ll spend a lot of time at our Day Center. That’s why it’s a bright and welcoming place - we’re happy you’ll be spending time with us!
Medical Services
All the medical and long-term care needs you have can be met through Central Valley PACE. Together with you and your family, your Care Team will craft a personalized plan to meet your needs.
Home Care
Personalized care in your home from fully-screened and medically-trained staff. Our home care staff can visit once a week to every day, depending on your needs.
Transportation
Our dedicated transportation coordinator will ensure you are transported safely to any off-site medical appointments in one of our handicap-accessible vehicles.
